Type: Preferred Provider Organization (PPO)

Protection services: 100% covered when connected to the network; It includes two oral exams and routine cleanings each year and one set of x-rays per year.

Basic services: 60% covered when connected to the network; Includes immediate care for pain relief and two fillings per year

Extras: $100 per year for in-office teeth whitening

Annual discount: $50 for an individual and $150 for a family discount
